Cardiac Rehab

Cardiopulmonary Rehabilitation is the process by which people with cardiovascular disease and/or pulmonary disease are restored to and are maintained at their optimal physiological, psychological, social, vocational, and emotional status.

This program was designed for two different populations because of the unique and individual needs of each of the following patient types. Participation does require a physician referral.

Cardiac patients include (but are not limited to) those patients who have been diagnosed with or have received the following procedures within the past 12 months:

  • Stable Angina (documented)
  • Myocardial Infarction
  • Coronary Artery Bypass Grafting
  • Heart Valve Repair/Heart Valve Replacement
  • Percutaneous Coronary Intervention (angioplasty/stent placement
  • Heart Transplant

Pulmonary patients include (but not limited to) those patients who have a diagnosis of:

  • Chronic Obstructive Pulmonary Disease
  • Pulmonary Fibrosis
  • Chronic Respiratory Conditions
  • Post Lung Surgery

Services Offered:

  • A medical evaluation to assess your needs and limitations in order to tailor a rehabilitation program for you and help you set goals.

Duration of program is assessed and determined by individual needs, diagnosis, and/or your progress. Please note that some payment sources define program duration and restrictions.
